                Participation medals will be given to wrestlers who do not place

            Weights:         70 75 80 85 90 95 100 105 112 119 126 133 140 HWT

Intermediate: 7th & 8th Grade or born in 1994-1995 Match Length: 1 1˝ 1˝

            Weights:         75 80 85 90 95 100 105 112 119 126 133 140 145 150 160 HWT

I _______________________, Parent or Legal guardian, and my family do hereby declare that, if I am accepted as a participant in this tournament by the Hopatcong Jr. Chiefs, I enter at my own risk and my own free will, and we understand and as herein identified, will not in any way hold liable Hopatcong Recreation, Hopatcong Jr. chiefs wrestling team, the Hopatcong School district, Hopatcong High School,Tournament officials, referees, coaches or any other personnel associated with this wrestling tournament for any injuries that I may receive, of any and all losses that I may incur, directly or indirectly, from training for, traveling to or from, or PARTICIPATION in this wrestling tournament.  I understand if my child has any suspicious skin markings, he might not be allowed to enter the tournament without a doctor’s note stating the wrestler is free of any contagious skin condition.

Coaches,

All criteria is from previous year (2008) tournament finishes.  You must use criteria number on registration form!!!!! If wrestler does have criteria then put 15 as your criteria. If no criteria is on the form then the  wrestler  will not be seeded. No changes will be made the day of tournament. THERE WILL BE  NO EXCEPTIONS.   For wrestlers who placed in qualifier you must specify which qualifying tournament the wrestler placed in.
